What is the client service program?….
This program provides clients with emergency relief for utility and mortgage/rent payments. Clients meet one-one-one with our Case Manager, who assists clients in providing funds for these services. The Emergency Medical Services program assists individuals by providing funding for clients in need of prescription drugs, eyeglasses, and medical supplies and equipment. Clients will make appointments with the receptionist to meet with Case Manager.Case Manager has up to (3) appointments in a given day. (i.e., 9:30 am, 11:00 am, and 1:00 pm)
Walk-in Clients may meet with Case Manager at the Case Manager’s discretion. If the Case Manager can not meet with-in client, they must schedule an appointment for a later date.
Client must bring all required documentation, and fill application with Case Manager. Case Manager and Executive Director evaluates application and provided information to make final decision to assist client.
Social Security cards for each member of the household
Proof of Income
Food Stamp Printout (if applicable)
All Bills (utility, rent/mortgage, car payments, loans, insurance payments, etc.)
****Client information is to be stored in a safe and secure place (i.e., a locked file cabinet). Client information is not to be shared or discussed outside of client services.
